I have read over the extension policy on Studio Dovana’s website. I acknowledge that hair extensions are very sensitive and different from intact human hair. I have been informed of the daily maintenance procedure to keep my extensions and actual hair in the best condition possible.

In the event I decide I no longer want to keep the hair extensions I am fully responsible for the total payments of removal. I understand that if an allergic reaction occurs, I will not hold my stylist at fault. I understand hair loss and breakage is a risk that can occur with wearing extensions.

Deposit agreement *

I undrerstand my non-refundable deposit is due before my hair will be ordered. My stylist will provide a confirmation of the type of the extensions to be placed, the length and amount of hair, the estimated number of hours for my service, the total cost of the extension service, and the required deposit amount.
